Summary

We are looking for an Adult's Social Worker to join a Hospital Reviews Team

It is REQUIRED to have a Social Work Qualification with at least 2 years of post qualified experience.

About the team

The team is required to undertake care act assessments and reviews, CHC checklists and MCA'S. Additionally, they review services that range from domiciliary care, supported Living, 24-hour placements, extra care housing and day services. As well as this, they use an extensive knowledge of the Care Act 2014, Mental Capacity Act 2005 and can undertake best interest assessments.

About you

Experience working within a hospital and or review team with continue health care will lend well for this role. A degree within Social Work (Degree/ DipSW/CQSW) with a minimum of 2 years post qualified experience is essential. A valid UK driving licence and car is essential for this role.
