Summary


The Band 3 Healthcare Assistant will work across healthcare and/or social care teams.

He/she will provide planned care with proximal supervision from a registered practitioner to deliver a seamless service to patients. The Band 3 Healthcare Assistant will work with other team members embracing case management and the long term conditions agenda thus preventing avoidable hospital admissions ad facilitating appropriate earlier discharges into the community. He/she will be proficient and competent to work in Community Nursing teams and with Community Matrons as deemed appropriate to their specific role. He/she will work within defined protocols and be able to take appropriate action to deliver.

Our working hours/days are as follows:
• Monday to Friday (Core hours) - 08:30 – 16:30
• 1 weekend every 4 weeks with days off in the week
• Late shift – 11:00-19:00
• Early shift – 07:00 – 15:00

Clinical

• Deliver clinical and therapeutic care to patients within agreed trust policies/ protocols, as documented in the band 3 competency assessment tool.

• Contribute to the holistic assessment and management of patients care needs throughinitialdata collection, monitoring ofpatientsprogress, feedback discussion reports and in care reviews with the Community Nursing Team.

• Implement and evaluate nursing plans in response toidentifiedneeds of the individual,carersand community in partnership withclients

• To provide care according to the available evidence of best practice, under the direction of a registered nurse following the BHFT delegation policy.

• Toidentifyand plan preventative measures and anticipatory care needs in discussion with registered nursing staff.

The “must haves” for this role:
• Car Driver with UK licence and use of a vehicle for business purposes
• Experience as a HCA within a medical healthcare setting
• Positive and motivational attitude
• GCSE/NVQ Level 3 Maths and English
