This is an experienced practitioner role, expected to undertake cases independently and demonstrating an ability to adapt the approach from first principles to deal with more complex or unusual case, assessing and managing risks appropriately. The role is for Hospital Social Worker. We are looking for someone with hospital Social Work experience, able to assess and facilitate safe discharges in a timely manner. This is a fast-paced team working collaboratively with health partners, housing, and other stakeholders to facilitate hospital discharges from acute settings. The management and team are very supportive. The team employ hybrid work with some days working from home depending on the needs of the service. The candidate should be able to work with children (see the adult, see the child) and young people, families, carers, and communities to help them express their needs and make informed decisions and contribute to service planning.
