Summary
A Vacancy at South London and Maudsley NHS Foundation Trust.
This is a new post created following the success of a pilot CESR fellowship. The post is based at the Bethlem Royal Hospital (BRH) and is full-time (9am-5pm, Monday to Friday). As part of the Portfolio (formerly CESR) fellowship, the post holder will spend two clinical sessions with either the Croydon Liaison Team (based at Croydon University Hospital), a Croydon community mental health team or a Croydon Inpatient ward. The professional development sessions will allow the post holder to develop their own interests within general adult psychiatry as they move towards working at a consultant level.The Portfolio (formerly CESR) fellowship is an opportunity for candidates for whom the traditional training route to a substantive consultant role (CCT) is not appropriate. The Croydon Home Treatment Team (HTT) is a well-established service within the South London and Maudsley NHS Foundation Trust (SLaM)
• Team Composition: Around 30 clinical staff, including psychiatric trainees, specialty doctors, and consultants.
• Caseload: Manages about 40 patients aged 18-65 with severe mental disorders, primarily referred from community mental health teams, crisis services, or inpatient wards.
Key Clinical Responsibilities:
• Manage the team's day-to-day caseload.
• Assess new patients within 48-72 hours.
• Conduct follow-up medical reviews and early discharge assessments.
• Attend daily team handover meetings.
• Perform risk assessments and formulate care plans.
• Monitor patients' physical health.
• Liaise with GPs, social services, and other professionals.
• Make safeguarding referrals and maintain accurate records.
• Carry out Mental Health Act and Mental Capacity assessments.
General/Managerial Responsibilities:
• Participate in service evaluation, audit, research, and quality improvement.
• Engage in service development and clinical governance activities.
• Attend team and Trust meetings.
• Contribute to academic activities and reflective practice sessions.
Support and Supervision:
• Regular clinical supervision from the Consultant.
• Annual appraisal and job plan meeting.
• Access to study leave and CPD opportunities.
• Compliance with mandatory training requirements.

Croydon Home Treatment Team Portfolio Fellow
• Location: Croydon Home Treatment Team (3 days/week) and a linked team (1 day/week).
• Special Interest Day: 1 day/week for non-clinical learning and special interests.
The team is made up of:
- One full time Consultant
- One full time CESR Fellowship Doctor
- One part time, two days per week, SpR
- One Portfolio (formerly CESR) Fellowship doctor [this post]
- One clinical service lead
- Four band 7 nurses
- Thirteen band 6 nurses
- Five band 5 nurses
- Three band 4 workers
- One peer support worker
- One business manager
- One psychological therapist and two psychology graduates.
There is a dedicated doctor’s office and has space for small MDT meetings and other shared clinic and office rooms that can be booked for use. There is also dedicated Admin support for the team.
The post will be supervised by Dr Andrew Howe, General Adult Consultant Psychiatrist for the Home Treatment Team.
Candidates can also join the SpR on call rota if they wish to if they are section 12 approved status. The on-call rota is non-resident, covers the borough of Croydon and involves working at Croydon University Hospital Liaison Psychiatry Department, the Bethlem Royal Hospital site and answering telephone enquiries.
