Summary
This role is varied, interesting and crucial to ensuring excellent patient experience every step of the way. This role will support the service in ensuring patients receive a high quality of care throughout their journey with CAMHS Services.
Perform specialist personalised, assessment and higher-level discussion of young people’s holistic needs, resulting in personalised care planning and evaluation of care delivery using advanced clinical knowledge and clinical skills.
Demonstrate higher level clinical care by managing a patient caseload providing expert assessment, planning, and treatment.
support the Day Hospital Service (within Specialty).
Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements, including providing access to community support.
Establish and maintain effective communication with young people and carers / relatives.
Support young people and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery.
Recognise changes in young people’s condition that require the intervention of others and case manage/refer on as appropriate.
To support, assist and encourage the multi-disciplinary team in resolving young people’s problems, by the provision of new and innovative models of case management.

• Acts as a role model demonstrating high standards of care and providing clinical leadership to others.
• Promotes a culture that supports self-management.
• Provides specialist education and training to other professionals involved in young people’s care.
• Leads the development, management and promotion of the service ensuring the delivery of high quality, cost-effective care.
• Embeds patient and public involvement within the sphere of practice.

• Develops new skills in response to emerging knowledge and techniques.
• Works across professional boundaries using creative reasoning and problem-solving.
• Instigates and manages change within a complex environment.
CLINICAL
• To participate as an effective member of the MDT and be responsible for the development and implementation of a patient centred approach to care: assessing, developing, implementing, and evaluating the care needs of patients and their careers.
• To ensure that statutory and legal requirements of care are incorporated safely within each young person’s programme of care.
• To observe the requirements of the Mental Health Act 1983 ensuring the patient is fully informed of their legal position and their rights under the Act.
