A Vacancy at Rotherham Doncaster and South Humber NHS Foundation Trust.


The Home First Service comprises of Occupational Therapists, Physiotherapists, Nurses, Community Mental Health Nurses and Rehabilitation Assistants. The service covers the Rapid Response service, Unplanned Physiotherapy, Occupational Therapy, Enhanced Care Home and the Hospice.



The Rehabilitation Assistant will support the qualified professionals and contribute to the assessment, implementation and delivery of care within patient’s homes/care homes.



RDaSH are offering this post along side the opportunity for you to complete an apprenticeship qualification [relevant to your role]. As an apprentice within RDaSH you will be given the opportunity to gain experience, learn work-related skills and obtain all the tools needed to thrive.

An apprenticeship includes regular training with a college or other training organisation. Around 20% of your working hours will be spent training or studying. This leads to a nationally recognised qualification.

Whether you already work for us or are joining the Trust and NHS for the first time, apprenticeships can help you to develop the skills, knowledge and know-how in order to embark on a successful and fulfilling career within the health service.

Undertake agreed activities with patients / service users under the direct / in-direct supervision of the qualified staff. This will include:

Complete vital sign checks, bloods and ECGs.



- Provision of mobility equipment/aids



- Provision of compensatory equipment i.e. raised toilet seats/bathboards



- Joint visits to support colleagues during moving and handling assessments



- Supervision of patients whilst completing rehabilitation programmes, along with training of care home staff where appropriate.



- Review the patient using standardised therapy and nursing assessments



- To report progress of the patient and changes in condition to the appropriate person.



- To ensure accurate documentation of care within the electronic record keeping programme



- To develop clinical competency and skills through the completion of competency based training, and to maintain skills through regular re-assessment and training



- To demonstrate sensitivity to the patient and carer’s needs by respecting privacy and dignity, whilst maintaining professional boundaries at all times.



- Develop a rapport with patients/service users/care homes and encourage them to participate in the rehabilitation / healthcare programmes and activities
