Community Care Officer SC6
Description
As a Community Care Officer at our organization, you will play a crucial role in ensuring the well-being and care of our community members. You will work closely with individuals and families to provide support, guidance, and assistance in accessing the necessary services and resources available to them. Your empathetic nature and strong interpersonal skills will be essential in building trust and rapport with community members, as well as in developing and implementing personalized care plans. Additionally, you will collaborate with internal and external stakeholders to advocate for the needs of our community and promote an inclusive and supportive environment.
Responsibilities
Conduct assessments to identify the specific needs and goals of individuals and families
Develop and implement personalized care plans to address identified needs
Provide ongoing support, guidance, and advocacy to community members, including accessing necessary services and resources
Collaborate with internal and external stakeholders to promote an inclusive and supportive community environment
Foster relationships with community organizations and service providers to enhance available resources
Regularly evaluate and update care plans to ensure the effectiveness and appropriateness of interventions
Provide crisis intervention and support during emergency situations
Requirements
Bachelor's degree in Social Work, Psychology, or related field
Proven experience in a similar role, preferably in community care or social services
Knowledge of local and national resources available to individuals and families in need
Strong interpersonal and communication skills, with the ability to build trust and rapport with diverse populations
Ability to assess needs and develop individualized care plans
Working knowledge of relevant legislation and regulations related to community care services
Excellent organization and time management skills, with the ability to handle multiple tasks simultaneously
Flexibility to work evenings or weekends as needed
Hours Per Week: 36.00
Start Time: 09:00
End Time: 17:00
Location: North London, London
Pay: £17.54 per hour
