Summary


To lead, develop, evaluate, and formulate long term strategic plans to ensure a consistently high-quality clinical pharmacy and medicines optimisation service to the Intestinal Failure directorate at Salford Royal Hospital. To work with the multidisciplinary teams to optimise patient outcomes and improve medicines safety. To lead on the teaching and education of medicines optimisation in Intestinal Failure across the pharmacy department. As a member of a National Reference Centre for Intestinal Failure, to provide teaching and education at a regional and national level.

Provides a specialised clinical pharmacy service to patients under the Intestinal
Failure Directorate as part of a multidisciplinary team.
·To oversee the clinical pharmacy service to the Intestinal Failure Directorate and
to lead on relevant medicines optimisation and safety initiatives within the
specialty.
·To plan and implement, under the direction of the Principal Clinical Pharmacist,
the strategic direction of the clinical pharmacy service provided.
·Undertakes risk management and ensures compliance with Medicines legislation
including national and local policies.
·Lead on relevant Intestinal Failure or general clinical training/ teaching/ education
sessions for pharmacists under the direction of the Principal Clinical Pharmacist.
·Acts as a role model for less experienced pharmacists.
·Dispenses and supplies medicines; provides medicines advice as required.
·Provides out of hours emergency pharmacy services including bank holidays,
weekends, lates and on-call.
·To form part of the Senior Clinical Pharmacy Leadership and Management team
and provide (along with other senior pharmacists) specified operational support
to the pharmacy department.
·To manage the Rotational Pharmacists in conjunction with other senior
pharmacists under the direction of the Principal Clinical Pharmacist.
·To develop and lead practice based research.
·To deputise for the Principal Clinical Pharmacist if required
