Summary


An opportunity has arisen for an enthusiastic and highly motivated DSN tojoin the innovative Barts Health NHS Trust Young Adults Diabetes Service.

This exciting new post has been developedto improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London.The post holder will support young adults who may be struggling with a range of difficulties related to living with diabetes, as they make the transition to adult services. The work will mainly be with young adults living with Type 1 diabetes using or about to start on diabetes technology, including insulin pump therapy.

The post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users.

The post will have a base at Mile End Hospital,but will involve working across all of the Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), and ensuring that standardised, high quality care is delivered across the Trust.

We welcome applications from qualified diabetes specialist nurses with a strong interest in working with this patient group.

In addition to being a member of the core young adult MDT the successful candidate will join the wider Barts Health diabetes service, andwill be well supported in this highly-regarded service, by more senior clinicians.
• Work across all sites at Barts health, with the young adult diabetes teams, providing clinical care.
• Attending MDTs for the young adult services across Barts Health
• Support patients converting to or commencing injectable and other therapies including insulin pumps and required to audit the outcomes of care against standards and initiate change as necessary.
• Contribute to the young adult clinical governance meetings
• Deliver structured diabetes patient education programmes.
• Contribute to the enhancement of diabetes nursing by acting as a credible role model, educator and expert clinician.
• Support patient peer support groups
• Provide clinical care and contribute to the development of training packages, interdisciplinary care pathways, protocols and audits.
• Provide education and support for primary and secondary care health professionals in relation to diabetes care.
