An opportunity has arisen for an enthusiastic & highly motivated DSN to join the Barts Health NHS Trust Young Adults Diabetes Service. This mobile service comprises of a Dietician, Clinical psychologist, Youth Worker, Social Prescriber, & administrator.

This post has been developedto improve diabetes care & outcomes for young people (age 16-25) with diabetes across North East London.The work will be with young people with all types of diabetes & will have a focus on the very high risk young people e.g those with a very high HbA1c, those that struggle to engage, those with learning disabilities, homeless.

The post holder will have scope to develop the service, with the support of more senior colleagues & in collaboration with service users, & to take a leading role in the future of diabetes services for young adults across North East London.They will be working with the young adult Social Prescriber and Youth Worker.

The post will be based at Mile End Hospital, but involves working across all of the Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), & ensuring that standardized, high quality care is delivered.

Along with being a member of the core young adult MDT, the successful candidate will join the wider Barts Health diabetes service, & will be supported in this highly-regarded service, by a more senior clinician.

The post is an exciting opportunity to join an innovative service that aims to improve diabetes care and outcome in young people with diabetes (both type 1 and type 2) across Barts Health. The post holder will be part of a core Young Adults diabetes team that will support diabetes transition and young adults services across Barts health.

The post holder will develop specialist skills enabling them to provide management plans, treatment advice, and support for young adults (age 16-25) with diabetes and their carers.

The post holder will work across all sites providing specialist care and support for these young people, their families and the site based clinical diabetes teams.

The post holder will be a clinical and educational resource for patients and health professionals; and will ensure a smooth transition from paediatric to adult care occurs in terms of diabetes care.

The post holder will be a good team player always striving to improve the care of people with diabetes.

The post holder will support the young adult diabetes youth worker and social prescriber
