Summary

Position of the role in Claire House The post holder reports to and is accountable to the Hospice Manager Leadership and line management as required for the service Member of the wider clinical leadership team The post holder will work collaboratively with the Hospice Manager, Hospice Inpatient Manager, Practice Education Team and Hospice Lead Nurses to ensure the delivery of a safe, effective and high quality service to all children and families Scope of Job Clinical lead and role model for clinical care team to provide safe, high quality care to all children, young people and their families Lead role and responsibility for clinical delivery of safe and effective care through teaching, training, auditing, reviewing, supporting and coaching Provide visible clinical leadership in practice Provide specialist nursing and clinical knowledge and advice to support and facilitate high quality, individualised care to children and young people in collaboration with the multi-professional team Be a Non-Medical Prescriber (NMP), contributing to the prescribing rota ensuring safe prescribing processes are adhered to Lead medicine management meetings to ensure practice is safe, effective and current within the hospice Support the Hospice manager in fulfilling the requirements of the Care Quality Commission (CQC), enabling children, young people and their relatives to receive safe, effective and high quality care. This role will take the lead on the Safe & Effective requirements Leadership & clinical collaboration and coordination with Rapid Response Team and Butterfly Suite Coordinators to facilitate end of life and neonatal care Investigation and management of clinical incidents and complaints, with actions and outcomes to continuously improve practice Lead role for the development and delivery of individualised care plans Recruitment and retention of a skilled hospice care team Duty Manager On-Call rota for Hospice Dimensions and Limits of Authority The post holder will have responsibility and accountability for the 24/7 clinical delivery of safe and effective care within the Hospice and reports to the Hospice Manager The post holder will take line management and mentorship responsibilities as required by the service. Appropriate advice should be sought from the Hospice Manager or HR Advisor Contribute to strategic service development in partnership with the clinical leadership team To work autonomously ensuring personal accountability in accordance with the Nursing and Midwifery Council (NMC) code of conduct. Designated lead for Medicine Management Qualification and Skills Level Significant clinical experience of Childrens Hospice Significant experience of working effectively in a leadership role within a childrens hospice or similar work environment Degree with evidence of relevant post graduate education Non-medical prescriber Proven ability to develop and deliver clinical policies and procedures Excellent interpersonal & communication skills Proven ability in line management and mentorship Proficient in core and extended clinical competencies Experience/ qualification in paediatric palliative and end of life care (EOLC) (degree level) Experience in the investigation and management of clinical incidents and complaints Extensive experience of developing and delivering individualised care plans Clinical Practice To be a highly competent, knowledgeable practitioner, ensuring the provision of excellent, evidence based nursing care for the children, young people and their families Provide specialist nursing and clinical knowledge and advice to support, teach and facilitate high quality, individualised care to children and young people in collaboration with the multi-professional team Ensure that each child has their individual needs assessed and their care is planned, implemented and evaluated in negotiation with the family and child/ young person where appropriate & that this is accurately documented and up to date Ensure in partnership with the Transition Coordinator that all care provided for young adults is appropriate and centred around their individual needs Ensure effective day-to-day communication in respect of childrens and young peoples health and care management with parents/carers and external health and social care professionals To provide knowledge and guidance in all aspects of Infection Prevention & Control (IPC) in the clinical environment, ensuring cleanliness and infection control standards are maintained Collaboration with the Referral Team, Respite Administrator, Hospice Lead Nurses (HLNs) and Rapid Response Team to coordinate and plan both planned and unplanned care for each child and family Highly competent and proficient in all core and advanced Claire House clinical competencies Contribute to the non-medical prescribing rota Act as a resource for clinical review including prescribing appropriate medication within the scope of individual practice Education and Training In partnership with the Hospice Lead Nurses foster an environment that is conducive to learning for both the care team and students Working in collaboration with Practice Education Team, identify, plan and deliver the training and development needs of care staff/ students in the hospice Leadership / Management Work closely and collaboratively with the Hospice Manager to act as positive role model for the hospice care team including volunteers Deputise for the Hospice Manager as required Provide clinical leadership and professional support to the care team to ensure the effective day to day management of the in-house hospice care provision Working closely with the Hospice manager maximise the use of staff resources within the agreed budget, including the efficient use of bank staff, ensuring all time sheets are completed as required Ensure safe levels of nursing care at all times, including the compilation of the off duty, notifying the Hospice Manager and Director of Clinical Services of staffing concerns that compromise patient safety Provide motivational leadership and support to direct reports and wider care team Preceptorship and mentoring of newly appointed care team members Lead, manage and develop the care team to work effectively and achieve objectives in line with the organisations strategic plan Lead and support the in-house care team through the process of change and innovation Champion Claire House values across the organisation by acting as a positive role model for staff and volunteers Promote an open, supportive and responsive working environment, by maintaining regular contact with team members, listening and responding to concerns and provide or facilitate appropriate and fair support Promote a coaching culture within Claire House and specifically for the care team In collaboration with the Head of Workforce and Hospice Manager coordinate and participate in the recruitment and selection of new care team staff, ensuring all new staff receive an appropriate induction package and allocation of a mentor Implement systems to encourage reflective practice and clinical supervision opportunities Participate in senior staff and other clinical meetings Provide clinical leadership and professional support for all issues relating to Safeguarding and Safer Working Practice for both children and young adults, working to Safeguarding and MCA policies and procedures Undertake effective investigation and managements of complaints and clinical incidents within the hospice, ensuring that lessons are learned are shared within the care team Manage sickness absence and performance / disciplinary management in collaboration with the Hospice Manager and HR Ensure that all hospice care team staff adhere to Claire House policies and procedures Coordinate and participate as required in the duty manager on-call rota Ensure that accurate nursing / care records are maintained for all children, young people and families in accordance with professional standards / guidance on documentation Responsibility for Clinical Governance and Quality Support the Director of Clinical Services (as Registered Manager), Head of Care Workforce and Hospice Manager in fulfilling the requirements of the Care Quality Commission (CQC), enabling children, young people and their relatives to receive safe, effective &high quality care Contribute to clinical governance and quality across all clinical services, including clinical incidents and risk assessment and management Ensure the delivery of safe and effective nursing care through the development and implementation of service standards, audit, research, service user feedback, feedback from incidents and risk management information Participate in and /or coordinate the investigation of any complaints raised and ensure the implementation of any actions identified from these investigations Develop appropriate policies and procedures as identified by the Director of Clinical Services
