Central London Community Heath NHS Trust is proud to be collaborating with Herts and West Essex ICB to improve the quality of Advance Care Planning across South and West Hertfordshire. The offer is to put in place an Advance Care Plan and Treatment Escalation Plan for all consenting care home residents within South and West Hertfordshire.

It’s an exciting time to join us and lead the future of Advance Care Planning in Care Homes and we are seeking an enthusiastic and skilled colleague to join us as we build on our successful existing models of work. Our transformation plans over the next 12 months will include further integration with system partners including the local ambulance services, GP practices and care home partners and a greater emphasis on population health and reducing inequalities. This work is key to ensuring that residents in care homes have their wishes expressed and documented robustly to prevent avoidable hospital admissions and respect people’s choices to receive care in the place of their choosing. You will have opportunities to be involved in change management and the direction of the project in future to ensure its continuing success. Due to relocation of the previous post holder, this exciting opportunity has become available to the right candidate.

The post holder will be responsible for providing administrative support to Clinical Nurse Specialist and Project team including preparation of reports, letters, maintaining diaries and appointments and collation of notes/ minutes as required. It will also include uploading data to clinical systems as required and maintaining databases of contacts and collating feedback for stakeholders. The post holder will also have the opportunity to visit patients and care homes to implement Advance Care Plans under the supervision of the Registered Nurses working in the team, if this is an area that they would like to develop their skills in.

The role involves working closely with Care Homes, and other members of the integrated community teams. As such you will be required to have excellent communication skills and be confident to work using your own initiative as well as being an excellent team player.
