Summary
IES – Intensive and Enablement Team
The Intensive and Enablement Service focuses on supporting individuals aged 18+ with a range of needs, including but not limited to; Learning Disabilities, Autism and Mental Health.
Individuals referred to the service may be at risk of crisis, in a crisis, risk of hospitalisation, risk of placement breakdown or may require a period of enablement to build on their independent living and problem-solving skills.
The service also offers support to individuals upon discharge from hospital under the Mental Health Act.
Focusing on individuals’ strengths and resilience the support will be provided in their homes, or in the community to develop their daily living skills to improve their quality of life. Therefore, the outcomes we seek are supporting positive wellbeing, feeling good and staying well.
We work with individuals for an intensive time period of up to 6 weeks in which we carry out a number of visits to support them to achieve agreed outcomes. The support that we offer is flexible depending on each individuals’ need and their outcomes.
We will provide regular training opportunities to support you in this role and you will be supported to undertake a 26-week probationary period with regular meetings throughout this period to monitor progress.
