Summary

Clinical Responsibilities Be professionally and legally responsible and accountable for all professional activities including occupational therapy assessment, diagnosis and treatment of patients referred to Mildmay as an autonomous practitioner and in partnership with the patient/carer; obtaining consent in accordance with professional guidelines and local team procedures. Demonstrate and apply theoretical knowledge of acute, multiple, complex and long-term pathologies and impairments in the assessment and management of patients. Use a range of highly specialist clinical assessments and skills to ensure effective management of patients. Recommend appropriate interventions to promote independence and safety, utilising effective clinical reasoning skills and evidence-based practice. Communicate relevant medical/social information, assessment details, advice and recommendations across agencies following local and professional guidelines for sharing information and consent. This would include Consultants, nursing staff, GPs Social Services, other health care professionals and voluntary agencies Convey complex and sensitive information relating to patients using skill in situations where there may be barriers to communication i.e. use of interpreters, receptive/cognitive pathologies or excess noise. As and when required to carry out clinical risk assessment and complete risk management plans relevant to the patient, including risk assessments for equipment provided. Ensure management of clinical risk identified. Complete an individual care plan where indicated for patients who require input from integrated community services (bed-based or home based input). This may involve other multidisciplinary team members. Engage in joint patient assessment/ treatment with other members of the MDT as need dictates Provide specialist advice and education to patients, carers and relatives based on clinical evidence and identified needs Maintain up-to-date clinical written records and electronic database in accordance with local and professional standards; and also writing comprehensive reports on clients including discharge letters to medical referrers. Ensure safe, appropriate and effective provision, installation and demonstration of equipment. Utilise and further develop knowledge of minor/major adaptation provision and non-stock equipment availability and use. To be responsible for ensuring that the equipment and environment within the OT assessment kitchen is fit for use, including arranging annual service inspections where appropriate To be responsible for the safety and good up-keep of the environment and to identify and manage areas of risk and to bring these to the attention of the Risk Management Team. To report untoward incidents which occur within the hospital premises. Management To co-ordinate and plan with the MDT a structured therapeutic programme of therapies and activities and delegating accordingly. To meet Clinical Governance and CQC Standards where they apply to ward service delivery. To be responsible for the recruitment and line management of allocated staff. This includes clinical supervision, monitoring of sickness and performance, annual appraisals, training and supporting CPD. Education & Research Responsibilities: Provide training and education programmes as required in the form of tutorials and presentations. Be responsible for the regular supervision and written assessment of OT students To be leading on and producing audit and research in own area Keep abreast of evidence based and best practice. Demonstrate ability to critically evaluate current research and apply it to practice. Ensure good working knowledge of national and local standards, and monitor quality through participating in clinical audit, research projects and evaluation work as required. Personal: Maintain continuing professional and personal development by participating in regular in service training, supervision, reflective practice and attending study days and training, giving feedback to the team where appropriate to disseminate learning. To maintain a portfolio of CPD that can be demonstrated to the Medical Director or external agencies Maintain competency and proficiency in manually handling people and therapeutic manual handling.
