Summary


An opportunity has arisen for a motivated and enthusiastic Physiotherapist to join our newly developed Home First services within Doncaster. The current post would be based on the Intermediate Care wards Hazel and Hawthorn, however cross cover across the Home First Service may be required at varying times.

Intermediate care services in Doncaster are currently evolving into a Home First Service model. The Home First Services provide assessment, treatment, and care support to patients in dedicated bed-based units or in their own homes. The service aims to avoid acute hospital admissions and support early discharge, providing care in the community when possible. The Urgent Community Response service is an integrated, multi-agency pathway designed to prevent hospital admission. Community Physiotherapy involves assessment of patients in their own home or care home, for provision mobility aids and rehabilitation plans. The therapy team provides the rehabilitation and reablement to optimise patients’ independence thereby reducing their dependence on care support. The post holder will be required to develop skills that will support all areas within the Home First Rehabilitation pathways, which include:
• Urgent Community Response
• Community Physiotherapy
• Reablement
• Specialist Falls service
• Hospice
• Virtual ward
• Intermediate Care Ward

The successful candidate will provide physiotherapy intervention, as well as supporting the multidisciplinary team in assessment and development of a holistic management plan. We are looking for a forward thinking, adaptable practitioner who is capable of working autonomously, and maintaining judgement under pressure.

Clinical
• To provide specialist physiotherapy assessments over a range of conditions and complexities using advanced clinical reasoning to analyse clinical and non-clinical findings to form accurate physiotherapy
diagnoses and prognoses.
• To assess patient understanding of assessment and treatment in accordance with the Consent Policy, and to assist members of the team to work within the legal framework for patients who lack capacity to consent to treatment.
• To provide health education, health promotion and prevention advice.
• To manage a clinical caseload, organising this effectively and efficiently with regard to clinical priorities and use of time with access to expert support as required, both for self and more junior staff, within MDT.
• To formulate and recommend to others the best course of physiotherapy intervention and/or treatment programmes to meet the agreed goals set with the individual patient/or carer, using evidence based practice and a wide range of treatment skills and options.
