POST:Band 7Clinical Lead AHP, Bed Base Rehabilitation, Home First Services, Doncaster.

HOURS:37.5 hours per week (1.0wte). 7-day week service

We are looking to appoint an enthusiastic AHP with excellent leadership skills into this newly developed post within RDASH.

Intermediate care services in Doncaster are currently evolving into a Home First Service model. The Home First Services provide assessment, treatment and care support to patients in dedicated bed-based units or in their own homes.

As Clinical Lead AHP you will work alongside other management to provide clinical leadership and operational management within the Bed Base Rehabilitation. You will help with the triage, planning, prioritisation and review of referrals into the services, problem solving with the team any potential bottlenecks or issues with meeting service delivery targets to optimise patient flow. You will also be required to provide / coordinate therapy provision to the NWB / Transfer of Care Beds located in Care home within Doncaster.

We are looking for a highly experienced Band 6/7 Physiotherapist or Occupational Therapist with a working knowledge of intermediate care provision who has the vision regarding a home first model of service delivery. Experience in managing complex hospital discharges and community rehabilitation will be essential.

The successful applicant joins us at an exciting time as we transform services in order to provide seamless and responsive care and rehabilitation across Doncaster Place. Previous involvement in service development would be an advantage.
