Summary
To carry a clinical caseload that includes a varied range of conditions and complexity. To assess the impact of illnesses/ trauma on function and implement the required adaptations/ supports to facilitate a safe and timely discharge. To work as an autonomous member of the multidisciplinary team, contributing to team discussions and decisions about patient care pathways. Responsible for the day to day management of Assistant Practitioners and students, e.g. clinicalsupervision, appraisals, caseload allocation To plan and implement appropriate discharges through comprehensive holistic assessments and knowledge of discharge pathways. To work in collaboration with patients/ families, carers, MDT, other workers and agencies across health, social care and the private sector to avoid admission or minimize length of stay. To complete accurate and timely documentation in accordance with REACTs Documentation policy. To collate, interpret and analyse activity data and report on findings for service development. To provide specialist reports on patient and service related issues as required. For further details please see the attached job description.
