This is an exciting new time to join the developing and evolving CYP Crisis service covering Wigan and Bolton. The team support CYP/families /carers in mental health crisis for between 72hrs and 6 weeks on two separate pathways. We provide swift risk assessment and management and brief collaborative, holistic, therapeutic interventions for CYP in the community to reduce the need for hospital admission or A&E presentation. You will be expected to undertake the full range of assessment and casework management for young people with moderate to severe mental health needs in the community, adopting a recovery and social inclusion model that encompasses non health and social care services to meet service user needs.

Service delivery

The service currently operates 7 days a week. Monday to Friday the operating hours are 8am - 8pm (including bank holidays) and on Saturday and Sunday 9am - 5pm. As the service develops there will be potential for extending these hours of operation.
• Undertake the full range of casework for people with moderate to severe mental health needs adopting a recovery and social inclusion model that encompasses non health and social care services to meet service user needs.
• Provide appropriate cover in the absence of the Team Manager as necessary.
• Assist in the development of collaborative care plans for clients / patients and undertake clinical activities as appropriate, ensuring a high standard of care.
• Ensure all appropriate client / patient and departmental records are maintained to the required standard and in line with CPA policy.
• Liaise with the wider multi-disciplinary team to enhance the standard of care clients/patients receive.
• Work within the framework of the Care Plan Approach
• Participate in risk assessment and formulation of risk management plans.
• Participate in staff inductions to develop the skills and knowledge of newly qualified nursing staff.
• Take a key role in communications within the multi-disciplinary team to ensure continuity of care.
• Promote and adhere to the Trust's values statement.
• Demonstrate high standards of Health and Social Care and be accountable for one’s own practice.
• Supervise and assess students during placements and other members of staff in the team, ensure appropriate advice and support is given

• Attend team meetings and care planning meetings as required, ensuring follow- up action is taken.

• Attending adequate training to ensure issues relating to service users and their families are assessed and addressed in relation to the policy, safeguarding Children.
• To act as Shift Coordinator within the Home Treatment Team
• To provide a range of interventions for service users to effectively manage any identified risks, to support them with their acute mental health needs and to support them to resolve crisis situations
• To develop a thorough understanding of deliberate self-harm, suicidality and management strategies using evidence based practice and NICE guidelines
• To observe and practice within the legal requirements of the Mental Health Act 1983.
• To contribute to the establishment and revision of policies, procedures and practice guidelines relating to the work of the Home Treatment Team and the interface between other Mental Health Services.
