Summary

As a key member of the PCN Multi-Disciplinary Team, you will provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes. You will develop trusting relationships by giving people time and focus on 'what matters to me'. Take a holistic approach based on the persons priorities and the wider determinants of health. Being aware of local support available to young people and their families, such as CAMHS, Wigan Youth Zone, and other young person services. Develop a good knowledge of a broad range of opportunities for young people to engage with their community and become involved in community-based activities. Take a holistic and flexible approach in your work with individuals supporting both social and psychological issues that arise. Provide non-judgmental and non-discriminatory support, respecting diversity and lifestyle choices. Work with individuals to co-produce a simple personalised action plan based on the persons priorities, interests, values and motivations including what they can expect from the groups, activities and services they will be connected to and what the person can do for themselves to improve their health and wellbeing. To work with patients virtually or face-to-face in the local community or in one of our 13 GP surgeries, to support and/or signpost them to local services/organisations, which the patient themselves feel would be of benefit to reach their goals. To provide service users with continuity and a coordinated experience of care, remaining the point of contact throughout. Manage and prioritise your caseload per the needs, priorities and any urgent support required by individuals and, where appropriate, refer the patient back to health professionals/agencies when the persons needs are beyond the scope of social prescribing. Build strong links with a wide range of local VCSE organisations, community and neighbourhood-level groups, utilising their networks and building on what's already available to create a menu of diverse community groups and assets that promote diversity and inclusion. Work sensitively with people, their families and carers to capture key information, enabling tracking of the impact of social prescribing on their health and wellbeing. Work closely within the Multi-Disciplinary Teams and with GP practices within the PCN to ensure that the social prescribing referral codes are inputted into clinical systems (as outlined in the Network Contract DES), adhering to data protection legislation and data sharing agreements. Identify risk issues that impact on peoples health or social care needs. Take appropriate action in accordance with the significance of the risk and be consistent with protection procedures, applying protection procedures, and following lone worker procedures. Produce tangible examples of best practice which can be highlighted and shared. Demonstrate effective team working inclusive of all relevant professionals. Report all accidents/incidents, ill health, and equipment and/or environment failures to line managers. Contribute towards audit and data collection as required. Adhere to policies and procedures within the PCN, including confidentiality, safeguarding, lone working, information governance, and health and safety. Work with your line manager to undertake continual personal and professional development, actively reviewing and developing the roles and responsibilities. Act as a key team member and support effective communication and stakeholder engagement and management both internally and externally. Build professional relationships with key staff in GP practices within the Primary Care Network (PCN).
