Internal ICB staff including managers, colleagues and senior management as appropriate Members of the public and service users as appropriate External staff from partner organisations and other NHS trusts Non-NHS organisations To work with provider services to assist in the development of competent assessors to undertake assessments and reviews. Manage day to day communication with other team members within the continuing healthcare team To provide contemporaneous records on all clients and reports as necessary adhering to NMC standards for record keeping. Responsible for collating evidence and information for the Decision Support Tool and liaising with the multidisciplinary team in order for a team recommendation to be made for eligibility for Continuing Healthcare funding.
