Job summary

Main duties of the job

What youll be responsible for:

  • This role identifies and assesses the primary healthcare needs of patients to determine whether they are eligible for funding either through Funded Nursing Care or Continuing Healthcare
  • Support teams to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively
  • Support and inform the targeting of resources, monitoring, implementation and evaluation of the tasks/projects by providing high quality support including complex information and analysis, communications and stakeholder management.
  • Internal ICB staff including managers, colleagues and senior management as appropriate
  • Members of the public and service users as appropriate
  • External staff from partner organisations and other NHS trusts
  • Non-NHS organisations
  • To work with provider services to assist in the development of competent assessors to undertake assessments and reviews.
  • Manage day to day communication with other team members within the continuing healthcare team
  • To provide contemporaneous records on all clients and reports as necessary adhering to NMC standards for record keeping.
  • Responsible for collating evidence and information for the Decision Support Tool and liaising with the multidisciplinary team in order for a team recommendation to be made for eligibility for Continuing Healthcare funding.
  • Present information for panel deliberation.

The role will involve, but not be limited to you performing the above.

  • Current 1st Level Registered General Nurse/NMC Registration
  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Knowledge and understanding of current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care
  • Awareness of current legislation in relation to Health and social Care for older people
  • Knowledge and experience of working with Safeguarding issues and Policy guidance
  • Experience of working with patients with multiple /complex needs
  • Experience of working across organisations, local authority, other NHS Trusts, voluntary and independent sectors
  • Successful experience working as part of a multidisciplinary team
  • Further training or significant experience in project management, financial management or supporting change management processes
  • Experience and understanding of evaluating and measuring the performance of health services.
  • Experience in communications and stakeholder management
  • Comprehensive knowledge of project management and/or health information systems development
  • Workforce development knowledge and experience
  • Working knowledge of Microsoft Project ECDL
