Summary

We are looking for a caring, dynamic and highly motivated manager to join the team as a Supported Living Manager in our Calderdale Support Services.
You will be based in one service which is located in Halifax, Calderdale that is home to 3 individuals.
You must be an accomplished practitioner with management skills and have experience and knowledge of working within the social care sector in support of people with learning disabilities, physical disabilities and complex health care needs (*referred to simply as ‘people’ in this JD).
Your role will be to ensure that the people you will be supporting receive the best possible care and support to ensure they can enjoy their best lives and reach personal outcomes. You will ensure that the care and support delivered is truly personalised and provided in accordance with agreed care plans, enabling people to enjoy wellbeing, quality of life and develop community connections

You will collaborate with the people you will support and associated health and social care professionals to provide a consistent and coordinated service that maximises personal outcomes.

You will work collaboratively with family members, all who are active key people in the lives of the people you will be supporting, ensuring open channels of communication on a daily basis.

You will support and supervise staff, and deploy staff resources effectively to meet the needs of the service.

You will be joining a progressive charitable organisation, strongly committed to investing in your professional development through both practice-based training and academic study. You will also be part of our highly committed and mutually supportive team of ten PBS and quality practitioners, supporting our agency to achieve great outcomes and meet quality standards.
