Summary
The Hastings Primary Care Network is in search of a compassionate and committed Frailty Specialist Care Coordinator to join our rapidly expanding team. In this role, the Frailty Care Coordinator will play a crucial administrative role, collaborating with practices and the broader community care team to ensure that frail patients receive appropriate interventions aimed at assisting them in maintaining as much independence as possible. This entails coordinating various clinician and social care services, as well as assisting individual patients in identifying and accessing support services provided by the voluntary sector or healthcare providers.
