Summary
As a care coordinator, your main roles and responsibilities include: Triaging patient referrals, Booking the home visits in on EMIS with the allocated clinician Updating the paramedic's visit sheets Route planning Updating patients on the status of visits Liaising with practices to update on the status of visits Answering calls and assisting with queries from both the practices and paramedics Liaising with the Business Manager as required to provide data Care coordinators are required to complete online training if not done previously as a requirement of the role. If training is needed this will be funded by the organisation.
