Summary


An exciting opportunity has arisen for a Support Worker to join our Cancer team at Luton & Dunstable Hospital.

This post offers an exciting opportunity to help shape cancer care pathways, patient support and management. The role is designed to strengthen existing supportive care services by providing support to the operational delivery and co-ordination of care for patients.

We are looking for a highly motivated individual who can communicate effectively and confidently whilst working intrinsically with MDT teams and colleagues within the acute setting and across the local community, both independently and as part of a team.

The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for managing their cancer journey. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.

The post holder will take an active role in contributing towards the development and implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive i.e. co-ordinating health and wellbeing clinics to enable independence and support self-management as appropriate.



The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.

1. JOB SUMMARY

The post holder will support the operational delivery of clinical services by taking an active role in supporting CNS and MDT teams with the support and management of care pathways for cancer patients.

Scope of Role:
The scope of the role will vary between tumour sites depending on the MDT priorities as determined during the set-up and development of the role. The role will have core elements that are applicable to all roles regardless of their setting and additional elements can be added according to local need.

The successful candidate will be responsible and accountable for their practice and behaviour under the guidance and supervision of a registered Clinical Nurse Specialist
