Summary
The Emergency Department at Nottingham University Hospitals’ NHS Trust is excited to be able to offer opportunities for motivated doctors to join our friendly ED team as Senior Clinical Fellow. We are looking for you to be able to start on 7th August 2024 (in line with junior doctor rotations) and the post will be for one year.
You will work as a Senior Clinical Fellow at Tier 4 level, which is our registrar rota. If you are an IMG with no NHS experience, we may start you on the Tier 3 rota as a Junior Fellow until you have had time to acclimatise and settle into the ED.
At the end of the year’s contract, following regular assessments and reviews, there may be the opportunity to apply for the CESR pathway.
We work with a variety of shifts which currently include:
0730-1730, 1000-2000, 1230-2230, 1400-0000, 1700-2200, 2000-0600, 2200-0800

This service provided by Emergency Medicine is based at the QMC campus of NUH. You will join a twenty-four hour a day, seven days a week roster, managing adult and paediatric emergency attendances.
Working in our ED encompasses many management skills such as managing the shop floor, keeping to the four hour target and constant reprioritisation of tasks.
Clinical Fellows in the Emergency Department can expect to:
• Build on previous experience and develop increased expertise in the identification, assessment and management of sick patients
• Gain experience and familiarity in identifying and managing a wide variety of medical conditions, with appropriated decision making around need for admission and discharge +/- primary care/specialty follow up
• Gain experience and familiarity in managing poly-trauma patients and the concept of ‘silver trauma’
• Have opportunities to acquire additional skills such as fascia iliaca block, chest drain etc
• To enhance your communication skills with patients relatives and colleagues
• To keep up to date with mandatory training required for your role
• To participate in the annual appraisal process
Our forward thinking Emergency Department is one of the busiest in Europe with 187,990 new attendances annually. The Children’s ED sees 47,019 children and young people and has recently expanded its resuscitation capability with a major capital investment.
The multi-professional Emergency Department team is key to achieving our vision of delivering ‘excellent care without exception’. A new leadership and governance structure, incorporating distributive leadership and ‘shared governance’ principles, aims to engage all staff in the achievement of this aspiration.
The nine bayed Resuscitation Area includes two adult and one paediatric major trauma bays, and the Majors Area has 30 monitored bays. In the Urgent Treatment Unit (UTU), ambulatory illness and injury patients are assessed and treated by primary care and ED clinicians.
QMC Emergency Department is the front door for the East Midlands Adult and Paediatric Major Trauma Centre (MTC) area and in 2022 saw over 1,479 major trauma patients, many of them older people, plus many more less severely injured patients. It is one of a national network of specialist centres which concentrate expertise and resources to give the best possible care, including intensive care and neurosurgery. The East Midlands MTC is the busiest in the country and has the strongest clinical outcomes nationally.
You MUST have completed MRCEM to be eligible for the role and have valid ALS and ATLS life support courses.
You will have at least 5 years post MBBS (or equivalent)
For doctors with NHS experience, at least two years working in an ED is essential
For non NHS trained doctors, three years in an ED setting is essential (an example of what we mean by an ED setting is working your entire job purely in the ED, not sharing the role with inpatient working/ward rounds)
Formal and informal educational delivery is a major strength of NUH ED and our DREEAM facilities support a wide range of training activities including the European Trauma Course (ETC). We offer daily timetabled supervision sessions from our ED consultants, enabling our staff to fulfil their portfolio competencies, as well as regular emergency floor “Education in the Fast Lane” multiprofessional, multidisciplinary education sessions. All CESR appointees are expected to support the education of the multiprofessional team in ED.
The Department has 24 hour Consultant cover, and the recent workforce investment has supported an increase across the entirety of our clinical establishment.

These are service posts so do not have educational approval, but you will be provided with excellent training and educational opportunities, tailored to meet your individual needs.
Where service provision allows, we encourage our fellows to attend educational teaching sessions.
We expect that you will keep yourself up to date with current knowledge and practice, attend clinical meetings within the department and to participate in audit and in service training of other professionals.
