Summary


As a discharge co-ordinator, you will be responsible for supporting your ward and the multi-disciplinary team in ensuring that our service users’ discharge needs are met through the delivery of high-quality care and information.

As a centre of excellence, you will offer a person-centred approach to their discharge planning and discharge needs. This will entail good communication and documentation.

Act as the focal point of contact for up-to-date information in relation to discharge, particularly for relatives and carers. Assist and provide good quality up-to-date, relevant written discharge information for patients, relatives and or carers. Develop an effective partnership with other wards, Hospitals and the Trust bed co-ordinators to co-ordinate the timely transfer of patients, at the earliest opportunity. Inform the bed coordinators of bed state changes as soon as the discharge coordinator becomes aware. Under the supervision of the nurse in charge, facilitate ward admissions by effective discharge planning.

Maintain a regular presence on the ward, attending all ward rounds, nurse handovers and multi - professional case conferences. Identify patients who could be discharged, enabling these patients to be reviewed first.
