An exciting opportunity has arisen for2 Registered Nursing Associates- Learning Disability to join the Isle of Wight Learning Disability Service.

Post 1) Full-time - Community Learning Disability Team

Post 2) Full-time - 15 Hours LD Liaison Team 22.5 Hours Community Learning Disability Team

The focus of the service will be to support adults with learning disabilities, reduce health inequalities, health facilitation and health promotion to people living at home in the community and supporting access to primary and secondary health services.

This is achieved by being an accountable professional, promoting health and preventing ill health, providing, and monitoring care with the RNLD, working in integrated teams and improving safety and quality of care for adults with learning disabilities.

The CLDT is island wide and based in Sandown, and the LD Liaison role is based in St Marys Hospital, Newport.

To provide and monitor safe, person-centred care to a designated group of patients, service users and families in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care.

Work alongside the registered nurses within the CLDT/LD Liaison to support the delivery of care and thereby creating capacity for nurses to commit to more complex aspects of care delivery.

To support the registered nurse in the contribution to on-going assessment and care planning for patients

To promote health and prevent ill health in patients and service users and contribute to integrated care.

To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required

The Nursing Associate will actively contribute to an effective learning environment and support others in their learning.

Nursing Associates will adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency.

Across Hampshire and the Isle of Wight healthcare services are evolving and transforming to meet the continuing care needs of our population. Now is a very exciting time to join Isle of Wight NHS Trust as we enter this period of change.

This year Isle of Wight NHS Trust and Portsmouth Hospitals University NHS Trust have strengthened our partnership and formed a group giving us an opportunity to work together to be innovative in improving patient care and outcomes as well as improving the experience and opportunities for our people across the two organisations.

We have also been working with other local NHS Trusts across our region to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the new organisation will be called Hampshire and Isle of Wight Healthcare NHS Foundation Trust.

As part of the move towards working as one organisation, people employed in our community, mental health and learning disability services by the Isle of Wight NHS Trust on 30 April 2024, will transfer to Southern Health Foundation Trust under Transfer of Undertakings (Protection of Employment) on 1 May 2024.

On 1stJuly 2024 Southern Health will become the new organisation (Hampshire and Isle of Wight Healthcare Foundation Trust).

The Isle of Wight NHS Trust will continue to deliver both acute and ambulance services for the island population.
