Job Description
As a Care Practitioner at Plas Dyfrynn, you will play a crucial role in improving the lives of our residents. You will provide assistance and support to service users who require care in a residential setting. This is a rewarding opportunity to make a positive impact on the lives of others.
Key Responsibilities
Attend to all residents’ personal care.
Monitor health and wellbeing of all residents.
Provide companionship and emotional support to residents.
Assist with meal preparation and feeding, as necessary.
Maintain a clean and safe living environment for residents.

Depending on qualification and training you will also be required to:
Administer medication and ensure compliance with medication schedules.
Document residents’ care activities in care records.
Collaborate with physicians, nurses, and other healthcare professionals to develop and implement Resident care plans.
Qualifications
Level 2 or 3 in Health and Social Care (Diploma or NVQ) or equivalent desirable but not essential.
Unqualified applicants must be willing to commence Level 2 in Health and Social Care which is to be completed within a 12 month period.
Registered with Social Care Wales.
