Outreach Family Support Worker
Description
We are seeking a dedicated and compassionate Outreach Family Support Worker to join our team. In this role, you will be responsible for providing support and assistance to families in need, helping them navigate difficult situations and connecting them to appropriate resources.
As an Outreach Family Support Worker, you will have a direct impact on the lives of families in our community. You will work closely with individuals and families to identify their needs and develop personalized plans to address those needs. This may include providing emotional support, helping families access healthcare and social services, assisting with budgeting and financial management, and advocating for families within the community.
To excel in this role, you should have excellent communication and interpersonal skills, as well as a strong understanding of community resources and social services. You will need to be able to work independently and make sound judgments about the best course of action for each family you work with. Additionally, you should be empathetic, patient, and non-judgmental in your approach.
Responsibilities
Conduct needs assessments and develop individualized plans for families in need
Provide emotional support and counseling to families facing difficult circumstances
Assist families in accessing healthcare, including scheduling appointments and connecting them with healthcare providers
Coordinate with community organizations and social service agencies to connect families with necessary resources
Assist families with budgeting and financial management, including accessing public benefits and developing savings plans
Advocate for families within the community and help them navigate systems and institutions
Monitor progress and evaluate the effectiveness of support plans, making adjustments as necessary
Requirements
Bachelor's degree in social work, psychology, or a related field
Proven experience working with individuals and families in a counseling or support capacity
Knowledge of community resources and social services
Excellent communication and interpersonal skills
Strong problem-solving and decision-making abilities
Ability to work independently and manage your own caseload
Empathy, patience, and a non-judgmental attitude towards families in need
Hours Per Hour: 35.00
Start Time: 09:00
End Time: 17:00
