Summary

He requires full assistance with personal care and activities of daily living. You will have the opportunity to develop your skills while delivering care to an individual in their own/family home. He lives with his family in an adapted Dorma bungalow and has access to an adapted vehicle to enable access to the community. You will undertake a range of duties to support him, working closely with other members of the team in providing the highest standards of health care. Excellent communication and interpersonal skills, backed by previous health care experience (preferred). Experience of supporting individuals who can display distressed(challenging) behaviours, however full training will be given to those with the right approach and attitude The post requires that you work independently, to a very great extent. The post therefore requires that you work and conduct yourself in a manner that is acceptable to the employer. Reliability, integrity and complete trustworthiness are essential attributes in this post. What you'll be working: 36 or 18hoursper week Monday to Friday 9am to 6pm What you will be doing: Establish and maintain a trusting, therapeutic, working relationship with client, family & Case Manager Support the client in line with the Care Plan guidelines Promote, monitor and maintain health, safety and security in the workplace Promote effective communication with all parties. Contribute to the protection of individuals from abuse. Liaise with other agencies when required Complete daily records as advised including; time sheets, daily records, care plan summary and critical incidents, petty cash etc Attend supervision, team meetings and other meetings with the employer and other persons as nominated by the employer Attend training sessions as arranged Support the client to attend and participate in a range of community activities. Support the client to attend hydrotherapy.
