This opening assignment is for 1-2 months

Hybrid working
Support clients referred into the service to prevent homelessness due to domestic abuse and provide person centred support and assist clients to secure and move in to independent accommodation.
Key Responsibilities


Act as key-worker for a number of clients staying in temporary accommodation (Refuge).
Undertake comprehensive assessments of referrals from Housing Options team, local DA Hub, and partner agencies including out of area, regarding their support and safety needs.
Assess and identify needs to develop and implement support plans based on a holistic and multi-disciplinary approach and complete comprehensive risk/safety assessments and plans in line with relevant Calderdale Safeguarding policies and procedures including the Calderdale DASH risk assessment.
Advise and assist clients to obtain the benefits payments, health and social care to which they may be entitled.
Ensure that residents comply with licence agreements and initiate action when a breach of the agreement occurs
Plan and arrange case meetings and reviews i.e. Child Protection conferences, and child in need meetings and act as ‘Lead professional’ where appropriate.
Assist clients to secure and move into settled accommodation and where necessary, continue support for up to three months (within Calderdale).
Liaise with the Housing Options Team and provide information necessary to determine the Council’s duties under homelessness legislation
Make the statutory decision to bring the Housing Act S193 main housing duty to an end.
Collect charges due for residents and strive to ensure that arrears do not accrue.

Qualifications:


Level 3 qualification in a relevant discipline or other level 3 qualification together with proven relevant experience.
Hold a full UK driving licence and have access to a vehicle for work purpose (adjustments can be considered for candidates with a disability)

Knowledge of welfare benefits and the application process

Experience of housing support work
Experience of working directly with adults who have experienced multiple disadvantage
