Summary


An opportunity has arisen for a suitably qualified and enthusiastic individual to join the newly established Stroke Rehab Bridging Service as a Band 4 Therapy Clinical Support Worker.

This role is part of a new team looking to improve the quality of the stroke rehab service by providing a seamless transition from Early Supported Discharge to Community Rehabilitation.

This role will suit someone who is passionate about providing high quality care and rehabilitation for stroke patients, working together with the MDT and in partnership with patients and families. The post holder will support patients to carry out rehab programmes set up by the ESD team and modify these programmes supported by the Senior Physiotherapist, Occupational Therapist and Speech and Language therapist in the Stroke Bridging Rehab service.

To be an integral member of the team Stroke rehabilitation Bridging service team, delivering therapeutic treatments to patients in liaison with senior therapy staff.

To take responsibility for designated elements of the patient treatment.

The successful post-holder must be an excellent communicator and motivator, experience of working within a Therapy setting is desirable.

Proven organisational skills is crucial. An ability to work in close cooperation, liaison and with the broader healthcare workforce is essential. They will have proven ability as a team player.

An ability to take responsibility, be flexible and have a sensitive and empathetic approach is needed. Participation in manual handling tasks is crucial and the post-holder must be able to undertake the physical and manual handling requirements of the post.
