Summary


This post offers an exciting opportunity to work within the Frimley Respiratory Team.

The post holder will work closely with other Respiratory Physiotherapists and Respiratory Nurses to support the virtual ward and provide specialist respiratory care to patients with long term illness. The successful applicant will be trained and supported to complete a structured competency ‘upskilling’ program by a Specialist Respiratory Physiotherapist. It is expected that competencies will be completed within 12-18 months (experience depending) when the successful applicant will move into a band 6 role. Support will be provided by the Respiratory multi-disciplinary team.

The physiotherapist will be responsible for the assessment and evaluation of care needs for people living with Respiratory Conditions, both in an outpatient and domiciliary setting. You will be part of the specialist workforce and use advanced assessment skills and knowledge to review these patients and support the Virtual Ward team to ensure the patients receive the same or improved clinical outcomes and patient experience compared with an equivalent inpatient stay. You will ensure that patients discharged from hospital following an admission for their chronic lung condition will have appropriate follow up to avoid readmission, receive appropriate advice and assess and monitor patients for home oxygen therapy. You will respond promptly to community requests for admission prevention for patients presenting with an exacerbation of their chronic lung condition. Part of this service will involve providing Respiratory Physiotherapy Services (chest clearance and breathlessness management) to patients experiencing an exacerbation of their condition. The post holder will also be involved in the running of the Pulmonary Rehabilitation Service across North East Hampshire, Farnham and Surrey Heath, particularly for those discharged from hospital or the virtual ward (Post Exacerbation Pulmonary Rehabilitation).

Communication
• Develop and maintain high standards of communication, including liaising with other health care professionals and clinical lead to ensure the safe and effective development of services to all respiratory patients on the caseload, but particularly to ensure the needs of Oxygen and COPD patients are met.
• Working closely within a multidisciplinary team of staff including Respiratory Consultants, Physiotherapists and Clinical Nurse Specialists in primary and secondary care, Advanced Clinical Practitioners, HCAs and Therapy Support Workers. You will also interact with GPs, Urgent Care response Teams, Community Nurse Palliative Care Specialists and other virtual wards.
• Within NE Hampshire, Farnham and Surrey Heath area act as a specialised resource on respiratory care and Oxygen use, to patients, health care professionals and the general public.
• Discuss with the team any referrals from GP’s, Hospital Doctors and Chest-Physicians and patients, to decide on the appropriateness of the referral. Using advanced communication skills communicate and liaise with patients and their families, chest physicians, GPs, Ward Nurses and Doctors, Palliative care Services, Health visitors and Community Nurses.
• Ensure effective communication by using interpreters when needed.
• Maintain up to date patient written and electronic records.
• Ensure that patients suffering from chronic lung conditions and those on oxygen therapy receive effective counselling /education. This often involves breaking bad news on diagnosis and prognosis.
• Develop an understanding of Trust and Board organisational structures and the structure of community services, and how to communicate with them.
• Maintain accurate documentation and audit in line with FH Clinical Governance Policy and HCPC guidance

Clinical
• Support patients with chronic lung conditions that have been identified by other health care professionals as needing to be monitored at home, thereby avoiding their admission to hospital.
• Visit patients suffering from chronic lung conditions in their homes to assess their condition, using advanced assessment skills; plan, deliver and evaluate patient care during their period of acute illness, and have advanced understanding of the complexity of issues surrounding the patient and their care.
• Support and review patients admitted to hospital that can be discharged home early with community / virtual ward support.
• Having evaluated outcomes, safely discharge patients from the service.
• Manage own caseload of respiratory patients and familiarise with all aspects of the Respiratory Service and Virtual Ward SOP/Clinical Flowcharts and Escalation Procedures.
• Take responsibility of organising and running Pulmonary Rehabilitation Classes
• Provide specialist clinical advice, dissemination of information and sharing of experiences with other health care professionals.
• Identify and rectify problems that arise during patients’ time on the service, liaising with appropriate personnel.
• Provide a respiratory service consistent with the latest Department of Health recommendations and according to local needs.
• Act as a competent and efficient decision-maker within and on behalf of the service.
• Be responsible for developing standards of care and monitoring clinical practice in partnership with the Frimley CCG.
• As part of the complex approach to admission prevention and supported discharge for patients suffering from COPD, ILD and Bronchiectasis, identify at risk patients and place them on the Virtual Ward monitoring to reduce the need for admission/ readmission.
• To always adhere to the HCPC Code of Conduct and Scope of Professional Practice.
• To contribute to the palliative care of respiratory patients as they approach end of life.
• To provide oxygen assessment and review to those patients referred to the service at home or within the clinic setting as per guidelines, considering the most appropriate modality should oxygen be required and providing risk assessment.
• Once trained, to maintain competence in using the blood gas and CRP analyser, and spirometry equipment
