The Virtual Ward is an exciting new service offering home based care to patients who would otherwise occupy a hospital bed. The care is delivered by a specialist workforce using face to face assessments and an integrated pathway to safely support patients in their own home. The developing Virtual Ward is looking at technology to assist with virtual assessment and monitoring to support the specialist in caring for people at home. We are looking for someone to provide a strong, visible, and effective advanced clinical presence for Nursing and AHP teams supporting the training and development of specialist and advanced practice. The post holder will ensure development of the virtual ward continues to deliver high standards, facilitating virtual ward care where appropriate and desired.

Operating closely with the clinical lead, the successful person will be based across Frimley Health facilitating the integration of teams within the respiratory service. Teams are currently based at King Edward VII Hospital, Windsor and Farnham Community Hospital, Farnham and rotate within the community and acute hospital sites – Wexham Park Hospital and Frimley Park Hospital. The role will be 3 days per week operational based at alternate community sites and 2 days per week clinical working at an advanced level at the acute site(s). The role requires flexibility in working patterns as the team is planned to cover a 7-day service.
