Summary


An exciting opportunity for a Community Assistant Practitioner has become available in the Downham Market and Swaffham Primary Care Network Team. This position is based in Downham Medical Practice to Cover the Fens and Brecks area.

As part of the Fens and Brecks Community Nursing Team you will provide interventions for patients in their own homes, care homes, and other community settings. You will be able to work autonomously and in an integrated way with our partners in Health & Social Care. The ability to work as part of a team and to have an adaptable approach to work is essential.

Working within the team will include a variety of patients, conditions and tasks including:
• Palliative Care
• Wound Care
• Long Term Conditions - i.e. Insulin administration
• Catheter and Bowel care
• Venepuncture

We offer comprehensive training, supervision and you will be supported by a a friendly and professional team, including an experienced Clinical Lead Nurse and Clinical Operations Manager.

The service operates between the hours of 08:00 – 20:00 and you would be required to work shifts between these times, as a Trust we will consider Flexible Working. If you want to work in a fulfilling team environment, then this is the job for you.





1. To contribute to the delivery of care packages for patients with long term conditions, palliative care, and chronic wounds for example, to achieve quality of life and independence where possible.
2. To work within the integrated team to facilitate early discharge from hospital
3. To work within the integrated team to prevent unnecessary admission to hospital
4. To work with Tissue Viability Nurse and Dermatology to gain experience in chronic wound management.
5. To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
