Summary
The Swaffham and Downham Therapy Team has an exciting opportunity to recruit a Community Assistant Practitioner, working out of the Downham office . The role as an autonomous practitioner involves patient assessment, goal setting, implementation of treatment plans and primary prevention strategies. The team cover a wide variety of conditions within a community setting, a current UK driving licence and access to a vehicle will be essential for this role.
You will need to be able to demonstrate effective communication skills, problem solving skills and be resilient and able to cope in changing and challenging situations. You will work under the direction of our Occupational Therapists and Physiotherapists, but will also be required to work autonomously. Previous experience of working within a community setting or within a therapy based team would be desirable. Patient visits in the in the community may be planned or unplanned depending on the need of the patient.
Supervision will be provided by Band 6 Occupational Therapist or Band 6 Physiotherapist. We offer the opportunity to work in an experienced and supportive team with opportunities for on-going professional development.

• Assess and treat patients with multiple conditions, sometimes complex in nature.
• Prepare for, carry out and monitor assessments and treatments in specified clinical areas, and discharge in line with predetermined department protocols.
• Modify and progress intervention / treatment using own clinical reasoning, notifying a qualified practitioner accordingly.
• Monitor patients and promptly alert a qualified practitioner when there are unexpected changes.
• Demonstrate problem solving, and contribute to the solution, working with colleagues.
• Make clinical judgements on patients’ response to prescribed treatment, making adjustments as required, and report findings to a qualified practitioner.
• Plan and prioritise own visits to patients and document on SystmOne ledger.
• Ensuring the outcome of patient visits are documented in SystmOne record within 24 hours of visit.
• Be flexible to accommodate admission avoidance patients.
