Summary


Acting as a Health Care Support Worker, working within a busy multi disciplinary team (MDT), providing rehabilitation and support to community patients as directed by the MDT.

Providing support to patients and families to prevent avoidable admissions and support discharge.

• Complete electronic patient records
• Report changes in patient condition to supervisors
• Under take patient observation
