Summary


An exciting opportunity has arisen for a full time , Band 5 nurse.

1 x full time 37.5 hours per week to join a well established Consultant Nurse led Tissue Viability Service (TVS) in Enfield Community.


This would be an ideal opportunity for the successful candidate to extend on their existing skills and develop further their knowledge and skills
within the tissue viability field. The successful candidate will assist in helping the team drive forward quality improvements in patient care.

The post holder will take a supporting role in the efficient running of the Tissue Viability Service (TVS). The post holder will be expected to provide
essential tissue viability advice to patients, carers and health care staff & professionals that is evidence-based.


The post holder will be expected to have fundamental knowledge and skills in Lower Limb and Pressure Ulcer prevention and management and
will be supported to develop further competence within a planned developmental framework. The post holder will work closely with the TVS senior nurses to deliver and develop their knowledge, skills and competence in this specialised area of nursing.

North Mid is part of North Central London integrated care system – consisting of the NHS and Local authority organisations in Camden, Islington, Barnet, Enfield and Haringey. As with other ICS’s, we are working increasingly closely with partners and indeed many of our financial and performance objectives are measured at this system level. Whilst all organisations remain as standalone, statutory bodies we have an ICS infrastructure for making shared decisions and agreeing shared approaches.

MAIN TASKS AND RESPONSIBILITIES:

CLINICAL PRACTICE(Patient Care - both direct and indirect)

1.1 Assess the patients referred to the TVS holistically, using a range of different assessment methods and reach valid, reliable and comprehensive patient centred conclusions, which manage risk and are appropriate to their need, context, culture and ethnicity.

1.2 Plan, deliver and evaluate programmes of care for patients referred to the TVS in conjunction with TVS seniors and the referrer.

1.3 Work with healthcare professionals in both primary, secondary, community, care homes and mental health services.

1.4 Communicate with patients in ways that empower them to make informed choices about their health and well-being and promote independence where appropriate.

1.5 To promote and deliver evidence-based nursing practice in relation to planning and delivering individualised patient care.

1.6 Be aware of and work within TVS and national and international policies and guidelines.

1.7 To provide essential wound care advice, as requested, for patients with tissue viability problems in local care homes, community teams, mental health units and other primary care settings associated with the TVS.

1.8 To deliver and promote clear, concise and specific documentation in relation to tissue viability issues.

1.9 To support district and community nursing teams, practice, ward and care home staff with the implementation of local and national practice guidelines, enabling them to initiate appropriate local developments and changes.

1.10 To participate in tissue viability interest groups locally.

1.11 To maintain professional networking relationships with all relevant professionals

1.12 To be accountable for one's own professional practice and professional development and ensure registration and compliance with the NMC and the Code of Professional Conduct, and actively participate in clinical supervision.

1.13 To take a supporting role in planning and delivering clinical practice in Care Homes, out-patient settings such as Leg Ulcer and Chronic Wound clinics.

1.14 Responsible for maintaining service and clinical stock


• EDUCATION AND TRAINING

2.1 To actively support the TVS to provide a learning environment that embraces a culture of evidence and research-based practice.

2.2 To plan the development, co-ordination and delivery of practice-based education and training in a variety of settings to different disciplines and levels of staff to facilitate the implementation of best practice.

2.3 To deliver and monitor learning outcomes of skill development workshops.

2.4 To monitor staff’s learning needs in relation to tissue viability and support and deliver the TVS programmes of education and skill development.

2.5 To ensure that information detailing study days and workshops provided by the TVS are advertised widely and ensure that access is organised and attendance is monitored, evaluated and reported.

2.6 To encourage the nurses who attend study days, workshops and courses on tissue viability issues to share their knowledge with colleagues at a local level and that practice is enhanced as a result of increased learning.

2.7 To maintain effective links between practice, management and education.

2.8 To support the delivery of the Tissue Viability Link Nurse system.


• RESEARCH AND AUDIT

3.1 To participate in clinical Tissue Viability research and audit projects and product evaluations.

3.2 To facilitate the implementation of clinical governance through the implementation of evidence/research-based guidelines, education and audit.

3.3 To support the dissemination of appropriate research findings to the clinicians within the Trust.

3.4 To support the organisation, co-ordination, delivery and evaluation of current practice in tissue viability management through the audit process.


• MANAGERIALincluding People (HR) Management

4.1 To manage junior colleagues within the TVS, undertaking Appraisals, Management Supervision Meetings, monitoring compliance with Trust and Professional expectations and standards (e.g. Administrative staff)

4.2 To support the collection, collation and reporting on service activities including outcomes data, audits, etc.

4.3 To take an active role in Quality Improvement approaches related to tissue viability.

4.4 Monitor and review TVS Asset Registers, Equipment maintenance and replacement and updates etc.

4.5 Participate in Clinical Supervision

4.6 Day to day supervision of junior staff/local induction

4.7 Deliver education and training sessions

4.8 Information Management: Responsible for the competent use of a variety of computer systems to monitor, create, maintain and report Trust and Service activity.


• Service Development

5.1 To work with the Consultant Nurse & Lead TVNs to pro-actively develop the TVS

5.2 Work with the Consultant Nurse & Lead TVN to ensure the TVS has a mobile working approach using up to date available IT platforms and software.


• Communication including Patient and Customer Care

6.1 Deal with sensitive or contentious information in addition to routine, complex or highly complex

6.2 Deal with fundamental staff issues or work planning/scheduling

6.3 Deal with problems that require analysis and/or interpretation in conjunction with senior staff.

6.4 Maintain details of day-to-day contact with patients/customers



7.0 Systems and Equipment

7.1 Responsible for stock and asset control and maintenance activity of service technical clinical and non-clinical equipment and equipment that relates to information management.



8.0 Effort, Mental and Environment

8.1 Working with people with chronic wounds requires a level of physical strength and skills often in challenging situations that may be tight for space, restricted light conditions, working at different heights e.g. kneeling on a floor, bed, couch, standing and bending over for protracted periods of time, depending on the patients’ location.

8.2 Working with people with chronic wounds and learners requires the ability to concentrate for long periods of time and complex mental effort is required.

8.3 Emotional support for patients and learners and colleagues and will require compassion, respect and the ability to tailor approaches to the presenting situations.

8.4 The job and role will involve a variety of working conditions, frequent interruptions, and exposure to substances that come under COSH regulations.
