Summary


You’ll play an integral part of a single PCN GP Support Team. Working alongside practices and other non-clinical and clinical roles, this team provides additional support and care to patients in an integrated way. On a day-to-day basis you’ll be supporting practices across your PCN with a range of activities relating to their patients. This can range from managing patient-related administrative requests through to identifying people who would benefit from greater proactive care, and organising/minuting regular multidisciplinary meetings . Plus, you’ll act as a champion for patient choice and empowerment making sure teams across the PCN have access to tools to support patient-centered care.

About you

You have excellent IT and communication skills. You’re a natural at organising and coordinating many different elements at once. You love to solve problems and take difficult situations in your stride. You take confidentiality seriously and have a common-sense approach, knowing when you need to get support or advice. Although this role is not patient-facing, you’re a compassionate person, who genuinely enjoys helping other people.

The main purpose of the team is as follows:
• Case Finding and Case Management to support clinicians
• Using population health intelligence to identify and work with a cohort of patients to access personalised care
• Managing the call and recall system for various GP targets
• Supporting efficient vaccination campaigns using IT tools available to you
• Administrating e-consultations
• Liaising and working closely with various PCN teams
• Managing requests relating to patient care
• Ensuring equity of access
• Coordinating monthly MDT meetings
• Building and maintaining great relationship with GPs across the PCN.
