About the Role:
As an Assertive Outreach Worker, you'll play a crucial role in delivering a top-tier, person-centred outreach service to individuals experiencing rough sleeping in Bath. Your mission? To make a positive impact on their lives and help them find stability.
Responsibilities:
Conduct assertive outreach and hot spot visits across the locality area, even during unsocial hours.
Maintain a regular street presence, connecting with those who need it most.
Perform risk and needs assessments with identified rough sleepers, ensuring they get access to suitable services.
Work within the No Second Night Out guidelines, especially when assisting those new to the streets.
Recognise signs of substance misuse, mental health needs, personality disorders, and behavioural issues, making appropriate referrals to relevant agencies.
Keep complete, accurate, and up-to-date client records using our client record system.
Requirements:
A valid UK driving licence and access to your own vehicle (business insurance will be required for roles involving travel for work).
