Summary
We are currently seeking a skilled and experienced Support Practitioner to join our team on a temporary basis. In this role, you will be responsible for providing a programme of person-centred support to customers referred into the Housing Related Support service in accordance with contract requirements.
Contract: Part time, 24 Hours per week , Fixed Term Contract of 12 months.
Key Responsibilities:
* To engage with customers on a weekly basis to support individuals to secure, maintain and achieve independent living.
* To support with the assessment of housing applications, identifying support, risk and housing needs using a housing first approach, ensuring signposting is carried out where relevant to other specialist/housing or support providers.
* To support colleagues with property allocation and lettings; carrying out accompanied viewings, ensuring effective ‘sign-ups’ for tenants whilst selling the ethos of tenancy sustainment and responsibility.
* To carry out regular property inspections utilising existing checklists and procedures to ensure that all properties meet compliance and maintain an acceptable standard. Chrysalis Housing Officers to take appropriate action with regards to supporting customers to report any concerns within their home including essential repairs and maintenance.
* To provide a low-level support package to customers to ensure they build/maintain the skills required to achieve independent living. Chrysalis Housing Officers are to utilise Humbercare’s support planning tools to identify areas of support required and work with customers to coproduce action plans to achieve.
* To ensure Chrysalis Housing properties are well presented and maintained by both our customers and Humbercare (in conjunction with the maintenance department).
* To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives.
The service is open to anyone over the age of 18 meeting the criteria and living in North East Lincolnshire. The Service is registered with the Care Quality Commission to deliver Personal Care. Shifts vary and may include evening and weekend work.
Essential Requirements:
* Educated to Level 3 Diploma or equivalent.
* Experience of working in a social care or social housing environment.
* Knowledge of tenant and landlord legislation
* Knowledge of statutory and voluntary support agencies.
* Experience of working with others to achieve successful outcomes.
* Excellent communication, interpersonal and problem-solving skills.
* Excellent time management skills and ability to effectively coordinate and manage a caseload.
Desirable Requirements:
Willing to work towards a recognised housing management qualification.
Current full driving licence and access to transport is desired but not essential.
Salary: £16,224.00 per annum - 24 hours per week - 12 month contract.
