Summary
To support service users referred to the Neuropsychiatry Service as well as their family/carers To provide one-to-one support in the home to people who have neuropsychiatric conditions To support people to access services To liaise with the family, relevant professionals and Neuropsychiatry Service Lead to ensure risk measures are indicated and managed Support and encourage volunteer feedback and conduct satisfactions survey to maintain quality measures Support and encourage service user/family/carer feedback and conduct satisfactory surveys to maintain quality measures Carry out cognitive functioning tests, ECG's and blood tests Utilise Older Peoples thrive menu to facilitate treatment plans
