Social Worker – Reablement Team
It is an exciting time to join Reablement, where we use a strengths based approach to promote independence and meet the health and wellbeing needs of vulnerable adults and their carers.
We are seeking an enthusiastic and hardworking Social Worker to join our Reablement Team, who cover the Wakefield District.
About the Role
You will work closely with key partners in Adult’s Services to carry out person centred assessments with adults across the Wakefield district.
Using a strengths based approach to assessment and support planning, you will:
Promote independence and self determination
Focus on positive change and prevention of harm
Support adults to achieve their personal outcomes
Undertake holistic assessments following a transfer of care from a range of settings.
The Team
The Reablement team is a district wide service offering short term reablement support to adults and older people over the age of 18 to help them maintain or regain maximum independence. The service is time-limited and supports effective discharge from the hospital settings and helps to prevent unnecessary admissions to hospital.
The case management team consists of 5 care co-ordinators, who complete reviews and statutory assessments of adults who require the reablement service.
There will be an expectation of line management duties such as case management and supervision.
Working Arrangements
Monday to Thursday:8:30am – 5:00pm
Friday:8:30am – 4:30pm
Flexible working arrangementsare available and can be discussed with the line manager
What We’re Looking For
As a Social Worker, you will:
Be passionate about making a difference and open to change
Demonstrate the ability to work under pressure
Be well organised and able to prioritise and manage risk within a complex caseload
Have experience and a strong commitment to strengths based practice
Be able to undertake standard social care duties, including:
Chairing meetings
Providing support and advice to junior staff
Producing high quality, person led assessments and reports (including court reports)
A full driving licence is essential, as the role covers the whole Wakefield District.

Essential Requirements
Applicants must:
Hold a Degree in Social Workor equivalent
Be registered with Social Work England
Have relevant post qualifying training and experience
Hold a full driving licence
