Summary


Enablement Pathway -

This is an exciting opportunity for experienced social worker to work within a dedicated, mental health, community enablement pathway.

This opportunity will utilise the specialism of social work to work within a rehabilitative framework of empowering and enabling, whilst maximising independence and adhering to a recovery focused outcome whether this be employment, training or sustained meaningful activity and routine.


The successful applicant will be required to provide highly specialist holistic assessments and interventions that are evidenced based with a comprehensive knowledge of relevant models.


Working in conjunction with team leads, service lead, and professional leadership, there will be an opportunity to focus on quality, service improvement, innovation, and research.


The post holder will be expected to support and promote a set of values that maximise the potential of individuals by promoting their recovery with the aim of reducing social exclusion, stigma and the potentially disabling effects of severe mental illness.


The post holders will offer a range of, outcome focussed, highly specialist social work interventions which will encourage positive risk taking and require working creatively, whilst providing recovery focussed care planning and achieving person centred recovery outcomes.


New service and pathways will significantly increase the capacity to provide timely assessment, treatment and support.

As a social worker in the team, we will ensure that you have clinical and professional support from a qualified Social Worker to support the maintenance of your professional registration. This support will be alongside the supervision and management that will come from the Trust giving the best of both worlds.

The successful applicant will work alongside the leads and colleagues to continue to develop the service and begin to pick up a specific relevant clinical caseload, and provide evidence based interventions as well as liaising with other agencies.

We are looking for enthusiastic, dynamic and experienced individuals who can work confidently on their own as well as being a team player. Good communication and problem-solving skills are essential, as well as knowledge of mental illness and forensic awareness. We also are looking for people who can demonstrate the Trust’s vision and values in practice, and are motivated to be involved in innovation and service development.
