About the role Our Clinical Nurse Specialists play an essential role in providing high-quality, person centred care for individuals facing life-limiting illnesses. Their role extends beyond direct patient care to include education, leadership, and advocacy. By combining clinical expertise, emotional intelligence, and leadership skills, they ensure that patients and families experience dignity, comfort, and peace during some of the most challenging times of their lives. Their role enhances the overall quality of palliative care and supports the healthcare team in delivering the best possible outcomes. This is a community based role, providing care in patients own homes and care homes You will act as an advisory and supportive service to external care providers, such as district nurses and GPs This is an exciting opportunity for an experienced Palliative Care Nurse who wants to work in a supportive environment where care of the patients comes first. Your main duties will be to provide direct assessment, support and advocacy to service users and carers dealing with complex, distressing and life-limiting illnesses. In addition: You may manage your own caseload and provide expert care to service users, appropriate to their needs, wishes and culture. You will collect, collate, evaluate and report information, maintaining accurate service user records.
