Summary
An exciting opportunity has arisen for a Patient Pathway Co-Ordinator/Admissions to work within an established team to provide administration support to the nursing team and prioritising admissions. You will act as the first point of contact in communication with patient queries over the telephone and provide a full administrative service to the nursing team.
The post holder should be proactive and have excellent communication, IT skills and be able to prioritise workloads for themselves to accommodate changing demands within the team and service. The role is very demanding and the successful applicant will be expected to be a core member of the Rheumatology team.
The ideal candidate will have previous NHS experience, a flexible approach to work, maintain a high level of professionalism at all times.
The post holder will oversee the patient journey to ensure, in conjunction with booking staff, that patient are scheduled in accordance with principles and target waiting times are set out in the NHS constitution, taking action to expedite patient pathway where necessary.

We run a Women’s Health Hub in Ilford; an Ageing Well Centre in Hornchurch; and Community Diagnostic Centres (CDC) at Barking Community Hospital and at St George’s Health and Wellbeing Hub. These CDCs are open 12 hours a day, 7 days a week.
