Summary


An exciting opportunity has arisen for aPatient Pathway Coordinatorto work within an established team to provide administration support to the clinical team and improve patient pathway.

You will act as the first point of contactin communication with patient queriesover the telephone and provide afull administrative serviceto the clinical team. The post holder should be proactive and have excellent communication, IT skills and be able to prioritise workloads for themselves to accommodate changing demands within the team and service.

The role is very demanding and the successful applicant will be expected to be a core member of the Rheumatology team. The ideal candidate will have previous NHS experience, a flexible approach to work, maintain a high level of professionalism at all times and experience in working in people facing environment.

Although this position is a 12 months fixed-term contract, the actual contract duration will be adjusted to account for the time taken to advertise the post, complete the hiring process, and finalise pre-employment checks. This means the contract will begin when the candidate starts the role and may be slightly shorter than 12 months.

To oversee the patient journey to ensure, in conjunction with booking staff, that patients are scheduled in accordance with principles and target waiting times as set out in the NHS Constitution, taking action to expedite patient pathways where necessary.

To provide administrative support to consultant/clinical teams including the management of clinician’sdiaries/timetables.

We run a Women’s Health Hub in Ilford; an Ageing Well Centre in Hornchurch; and Community Diagnostic Centres (CDC) at Barking Community Hospital and at St George’s Health and Wellbeing Hub. These CDCs are open 12 hours a day, 7 days a week.
