Summary


We are seeking a proactive and collaborative individual to support the delivery of transformation and change initiatives across the Care Group.

The role involves implementing changes within the care group aligned to the Trust Strategic Objectives and 28 Promises, developing workflows and processes, and supporting staff through the introduction of new technologies and equipment. You will work closely with clinical and corporate teams and build strong relationships with internal and external stakeholders to ensure smooth transitions and improved service delivery.

The post is fixed term until 31 March 2027.

The successful candidate will have demonstrable experience in change management or service improvement, strong communication skills, and a good understanding of healthcare environments.

Experience of reporting on and analysing data from systems to provide information/assurance in line with project requirements.

The successful candidate will support implementation of systems and processes to monitor and report on how we are achieving compliance such as SMI Physical Health Checks, S117 Reviews, Waiting Times etc.

The postholder will be expected to work at any location within the Trust footprint and will reach out to a number of partner agencies to deliver the project objectives.
