Summary

The Armed Forces Community Social Prescribing Link Worker (AFC SPLW) service, established in 2021, supports the health, wellbeing, and wider needs of the Armed Forces Community (AFC). This role works across Cornwall and the Isles of Scilly, supporting veterans, their families, and carers.

The AFC SPLW provides tailored, person-centred support, including 1:1 interventions for individuals with complex needs. Key duties include developing personalised care plans, connecting individuals to relevant services, supporting access to funding, and advocating to ensure sustained engagement. The role also involves delivering training to professionals and promoting veteran-aware practices across organisations, including GP surgeries.

Key Responsibilities

Assess referrals and deliver holistic, strengths-based support plans focused on individual goals, including employment and wellbeing.
Provide brief interventions (e.g. alcohol awareness, smoking cessation) and signpost to services such as mental health, suicide prevention, and domestic abuse support.
Empower individuals to build confidence, resilience, and independence.
Act as an advocate to help individuals navigate health, social care, and community services.
Collaborate with GPs, multidisciplinary teams, public health, and voluntary sector partners to ensure coordinated care.
Deliver training and mentoring to professionals to improve understanding of AFC needs.
Promote awareness and increase the number of veteran-friendly organisations.
Engage in community outreach to raise awareness and increase referrals.
Maintain knowledge of local services and support improved identification of AFC members.
Keep accurate records, contribute to data collection, and support service evaluation.
Ensure compliance with safeguarding, governance, and organisational policies.
Undertake ongoing training and any additional duties as required.

External: GPs, healthcare professionals, veteran organisations, community groups

Person Specification

Essential:

Understanding of the Armed Forces Community and related challenges
Experience supporting individuals with complex or long-term needs
Experience delivering 1:1 support and action planning
Strong communication and interpersonal skills
Ability to manage a caseload independently
Experience in health, social care, or voluntary sector settings
Good IT skills for recording and reporting
Full UK driving licence and access to a vehicle

Desirable:

Experience working with veterans or Armed Forces services
Experience with GPs or multidisciplinary teams
Community engagement or outreach experience
Experience working to targets or reporting outcomes
Knowledge of behaviour change approaches (e.g. MECC)
Relevant coaching or advice qualifications
Experience with data collection or service evaluation
